I understand that the information in this home care provider registry is provided in accordance with CT Public Act 24-39 and is intended to help members of the public find home care workers. The information in the registry has been submitted by home care agencies that employ these home care workers.
The accuracy of the information provided by a home care agency is the sole responsibility of that agency, and the State of Connecticut and its Departments are not responsible for any inaccuracies in such information.
The State of Connecticut does not endorse any home care agency or home care worker listed in this registry, and I understand that it is my responsibility to interview home care workers listed in this registry, check any references and credentials provided by such workers or their agencies, and make an informed decision about whether to hire any particular worker or agency. I understand and agree that neither the State of Connecticut nor its Departments are liable for any harm caused by a home care worker or agency listed within this registry.
